What to write down
One page per situation. Each opens with the whole answer in a sentence, then splits into the things to note and the things to capture before they are moved, cleaned, edited or deleted.
None of these pages says whether anything was allowed or what anyone can claim — that depends on the agreement and on the state. They say what a record of it needs to contain, because that part is the same everywhere.
